Sarepta Therapeutics – Elevidys's Clinical Standing & Next-generation Gene Therapy Approaches
Transcript key insights:
- Regarding Elevidys, the expert doesn't "perceive this as a situation where benefits clearly outweigh the risk, at least for the non-ambulatory patients." The expert also anticipates demand slowing in late-ambulatory patients and a not small population of historical skeptical families
- Expert anticipates Sarepta Therapeutics showing no deaths with new immunosuppressive regimen and FDA returning Elevidys to non-ambulatory market with a black box warning, and clinicians will "have to deal with it" without addressing underlying risks
- Exon-skipping therapies have been a "big disappointment" with limited perceived benefits compared to prednisone, and expert doesn't see even next-gen approaches as the winning strategy
- RGX-202, SGT-003 and Elevidys are more similar than different, expert says, with no clear winner due to limited understanding of dystrophin's function
- Expert expresses pessimism about significant breakthroughs in DMD treatment over the next five years, expecting only slow and incremental progress at a high cost

Avidity Biosciences – Del-zota Approval Outlook & DMD Payer Dynamics
Transcript key insights:
- Expert believes del-zota biomarker data is promising but cautions against over-interpretation without corresponding functional data to determine the quality of dystrophin expression, noting a possibility of FDA requirement for phase 3 study for approval
- Cardiac outcomes are crucial for evaluating new DMD therapies, as current exon-skipping treatments don't target cardiac muscle, which is life-limiting in DMD patients. "We could actually be in a scenario where we improve motor function so significantly that we accelerate the cardiac disease because individuals are moving more"
- Around a 3-point improvement on North Star Ambulatory Assessment would be considered clinically meaningful, though "if presented with Elevidys and an exon-skipping therapy that had comparable, very similar efficacy data, despite all the risks of Elevidys, I think a lot of people would still pick Elevidys"
- Durability might be a key differentiator for exon-skipping down the line or its potential for bridging patients to better gene therapies

The Next-generation Exon-skipping Therapies Shaking Up DMD Treatment
Transcript key insights:
- Recent deaths associated with Elevidys gene therapy have caused concern, and expert believes regulatory scrutiny will be harder on all following DMD therapies
- Dyne’s Force platform is viewed advantageously vs Avidity’s exon-skipping technology, with the key being in superior muscle targeting – "I believe the Dyne's clinical rating scales are superior, not by a big margin, though", but would use DYNE-251 in all amenable patients if approved
- Expert considers 3-10% dystrophin expression a good baseline goal for new treatments, with expectations increasing over time as therapies improve. 1-2% difference in therapies would be considered clinically meaningful
- Comparing efficacy across different exon-amenable therapies is virtually impossible. Expert doesn't put any weight in proposed comparisons and expects different therapy approaches may work best for specific mutations
- Expert predicts gene therapies will become the foundational treatment for DMD, with exon-skipping therapies used as sequential additive treatments to augment results
